Advance Directives

Planning your end-of-life care is a difficult subject that most people do not want to think about. However, it is important to plan in advance and to make certain decisions. Advance directives are a vehicle to assist you in end-of-life planning. In New York State there are three types of advance directives: The Health Care Proxy form, The Living Will, and Do Not Resuscitate Order (DNR.) 

A Living Will is a legal document that gives written instructions that explain your health care wishes, especially about end-of-life care. This document becomes effective when you are unable to make your own decisions.
 
A Health Care Proxy is a document that allows you appoint an adult to make decisions about your medical treatment if you lose the ability to decide for yourself. This person can make decisions to remove or provide life-sustaining treatment.

A Do Not Resuscitate Order (DNR) is a medical order written by a doctor. It indicates that a person – usually with a terminal illness or other serious medical condition – will not receive cardiopulmonary resuscitation (CPR) in the event his or her heart or breathing stops.
